Diabetes Self Management program is designed to help individuals manage their disease. It includes a diabetes assessment, with a Certified Diabetes Educator, to develop an individualized self-management plan.
This program is certified by the American Diabetes Association and the State of Michigan. This program offers both group and individual appointments.

When arriving at the hospital for the first appointment, please go to the Admitting Office located by the front lobby entrance. You do not need to register at the Admitting Office for any other appointments.
Insurance companies are recognizing the importance of Diabetes Education to help prevent long-term complications. Most insurance companies are covering a portion or most of the cost after your deductible is met. Check with your insurance company to see if they cover Diabetes Education. If you do not have insurance, please call to find alternative ways to receive Diabetes Education. We will be glad to assist you.

MMH Diabetes Support Group meets the monthly at the MMH conference room. We discuss various topics in diabetes care
and treatment. The support group is free of charge and open to the public. Please call for dates and times.
