What To Bring For Outpatient Services

• Complete Physicians Order – Patient Name, Birth date, and/or social security number, physicians name, test (s) ordered, and diagnosis. [Without a complete order we cannot perform services]
• Insurance Card
• Social Security Number
• Drivers License
• Authorization for Scheduled Services for HMO/Managed Care Patients
• Any Co-pays or Coinsurances that would apply – check your policy or your card for information.

What To Bring For Inpatient Services

• Insurance Cards
• Authorizations for Services (required by your insurance company)
• Advanced Directive or Patient Advocate Form
• List of Medications
